1. HEALTH HISTORY INTAKE
Welcome! This Health History Intake Form will help me understand your goals, body, and lifestyle so we can start working together to create your customized plan.
Follow the link to complete a thorough wellness intake covering:
→ Hormonal phase (pregnancy, postpartum, perimenopause, etc.)
→ Past injuries & movement goals
→ Nutrition, sleep, stress & lifestyle habits
→ What you want to return to (adventure, confidence, strength, more stability, etc.)
2. SCHEDULE YOUR 1:1 ASSESSMENTÂ
Now that you've filled out your Health History Intake Form, it's time to set up your 1:1 Corrective Exercise Assessment.   Along with the information I've collected from your health history, I’ll evaluate your posture, movement, and muscular imbalances. From there we'll put a plan of action together to create more balance and strength in your body as you work towards your personal goals.Â
This is a 90-min session is designed to help you pinpoint your muscular imbalances and teach you how to establish a strong core foundation, no matter what transition or stage of life you are in. Â
What to Expect:

A brief interview to go over your Health History Intake Form.

An assessment covering basic primal movements

Discussion on what's going to make the biggest impact on your body moving forward and where you should focus your efforts in your movement or fitness sessions in order to start building a stronger core foundation. Â

Brief coaching session with your focus in mind so you can start feeling the results right away.

Program and exercise recommendations to get you started right away!Â
What's Needed: You will need a mat or blanket and a set of weights. Choose a weight that would feel moderate to challenging in a bent row exercise. If you don't have weights, I'm the queen of improvisation! Water bottles, canned goods, a heavy laundry basket, etc work as well!Â
